I hereby give consent to Polyurethan LLC (630005, Russia, Novosibirsk, Musy Dzhalilja St., 9) and its duly authorized representatives to process my personal data as defined in the consent form and to any action (operation), including cross-border, or set of actions (operations) performed with my personal data including collection, record, classification, accumulation, storage, revision (update, alteration), extraction, usage of data, delegation, provision of access, depersonalization, blocking, deletion, termination of personal data with the use of automation facilities, including the use of information and telecommunication networks, and without the use of automation facilities, where processing the personal data without the use of automation facilities is appropriate to the nature of the action (operation) performed with the personal data with the use of automation facilities, and therefore permits searching activity, as per procedure, of personal data fixed on a material object and card index or other systematic collection of personal data, and/or the access to personal data in order to provide me with goods and services. I hereby confirm that the personal data (surname, first name(s), date and place of birth, home addresses, telephone numbers, passport details, email addresses) privided are correct and may be processed by Polyurethan LLC and its duly authorized representatives in order to implement the stated purposes.
Withdraw of consent can be addressed at any time by sending notification in writing to Polyurethan LLC (630005, Russia, Novosibirsk, Musy Dzhalilja St., 9).
